By checking this box, I confirm that I am the parent/guardian of the registered child and consent to their participation in Play Squash Academy programming.
I understand that squash, like any sport, involves physical activity and carries a risk of injury. I acknowledge these risks and agree that Play Squash Academy, its coaches, and partnering facilities (including Silver Creek School and OneLife Fitness) are not liable for any injuries, accidents, or medical issues that may arise during participation.
I also agree to ensure my child wears required safety equipment (protective eyewear and non-marking shoes) when applicable.
I consent to my child’s participation in Play Squash Academy programs and accept the above terms.
