WAIVER & LIABILITY AGREEMENT

1. Assumption of Risk

I acknowledge that participation in squash and related athletic activities at Play Squash Academy (“PSA”) involves inherent risks, including but not limited to:

  • Physical injury (sprains, fractures, concussions)
  • Collision with walls, equipment, or other participants
  • Fatigue, dehydration, or overexertion

I voluntarily assume all risks associated with participation.

2. Participant Health & Medical Consent

I confirm that:

  • The participant is physically fit to participate
  • Any medical conditions, injuries, or allergies have been disclosed

In the event of an emergency, I authorize PSA staff to:

  • Obtain medical treatment
  • Contact emergency services

I accept full financial responsibility for any medical care provided.

3. Release of Liability

To the fullest extent permitted by law, I release and hold harmless:
Play Squash Academy LLC, its owners, directors, coaches, staff, contractors, and affiliated facilities (including but not limited to OneLife Fitness and partner venues)

From any claims, liabilities, damages, or expenses arising from participation, including those resulting from negligence, except in cases of gross negligence or willful misconduct.

4. Code of Conduct & Safety

Participants agree to:

  • Follow all coaching instructions
  • Use equipment safely
  • Respect players, staff, and facilities

PSA reserves the right to remove any participant for unsafe or inappropriate behavior without refund.

5. Photo & Media Release

I grant permission for PSA to use photos/videos of participants for:

  • Marketing
  • Social media
  • Website and promotional materials

No compensation will be provided.
Written notice is required to opt out.

6. Acknowledgment

By registering for any PSA program, I confirm that:

  • I have read and understand this agreement
  • I agree voluntarily
